DescriptionThe Foundation is the philanthropic arm of the largest association of pharmacy professionals in the United States. The Foundation shares and supports its namesake organization's vision that medication use will be optimal, safe, and effective for all people all the time. The Foundation's Optimizing Technology Solutions Innovation Grant Program supports projects that demonstrate the impact of optimizing health information technology and digital transformation that enhance safe and effective use of medications. Priority is given to demonstration projects in health services delivery that are innovations and measuring outcomes, including enhanced effectiveness of care, and improved outcomes for patients, clinicians, and organizations. The deadline is for the Collaborative Care Grant for Nurses and Pharmacists. Visit the Web site to learn more and for application instructions.
