Description

The Foundation is the philanthropic arm of the health care Company, which assists healthcare systems and facilities across the continuum activate the clinical and supply chain resources needed to deliver their best care. The Foundation, committed to social determinants of health, is investing resources to help nonprofits address prevalent health challenges, stimulate health equity, and improve population health in underresourced areas. The Foundation's Community Impact Grant Program is designed to assist those living in vulnerable communities and those disproportionately impacted by social determinants of health. The Program works to address the economic and social conditions that influence individual and group differences in health status. Grants will be awarded to nonprofits that offer community-centered solutions, stimulate health equity, and improve population health in underresourced areas with the goal of addressing challenges in chronic diseases, breast cancer, and population health.
