DescriptionThe Foundation is the philanthropic arm of the physicians’ professional Association. Through the Foundation’s Community Health Program, grants of up to $60,000 will be awarded to community-based organizations with programs focused on the prevention and reduction of Type 2 diabetes and hypertension. Priority will be given to programs that employ evidence-based practices and that employ evidence-based practices and serve vulnerable or hard-to-reach populations. The Foundation will renew the grants for up to three years. Additional information can be found on the Web site.
