Description

The Foundation is the philanthropic arm of the healthcare supply chain Companies in North America. The Foundation’s E3 – Effectiveness, Efficiency, and Excellence in Healthcare grant Program supports projects developed by nonprofit healthcare institutions throughout the United States that help healthcare providers implement best practices to truly transform patient care. The focus changes from year to year, but generally includes some or all of the following criteria: medication safety at transitions across the continuum of care from hospital to home and ambulatory settings; taking successful work to scale; engaging patients and their caregivers and families; publishing or otherwise sharing outcomes, and finding innovative and sustainable ways to effect long-term change. The Foundation is willing to consider multi-year requests of $50,000 per year. Additional information as well as an online submission form can be found on the Web site.
