The Foundation’s Local Funding Partnerships supports innovative, community-based projects throughout the United States. Under LFP, local grantmakers propose a funding partnership with the Foundation to provide seed money for a promising, original project that can improve the health of vulnerable people in their communities. The Vulnerable Populations Portfolio is a diverse collection of innovative programs that addresses health issues within their broader social context through sensible, sustainable solutions that have the potential for scale. The Portfolio funds nontraditional solutions that improve health by meeting people where they are. Portfolio grantees work with “what is” and create new models of success: they restore what’s best and renovate broken systems. They renew the vitality of local environments and organizations. They redirect available resources and energies for better results. They re-engineer existing institutional solutions. And they re-imagine ways to create sustained improvement in quality of life and health for all, especially those most challenged by additional social factors. Eligible projects must be new, innovative, collaborative, and community-based. Significant program expansions – into new regions or to new populations – may also be considered. They must be nominated by a local grantmaker committed to participating as one of the funding partners. Local funding partners must be willing to work with grantees to obtain sufficient dollar-for-dollar matching funds throughout the grant period. The Foundation will award 12 grants for this round of LFP funding. Additional information can be found on the Web site.
