DescriptionThe Foundation is the philanthropic arm of one of the largest Medicare managed care plans. The Foundation is dedicated to improving the health and well-being of children, adults, and families throughout the state of Ohio. The Foundation's Responsive grants address one or more of the social determinants of health including: poverty, domestic violence, hunger, housing, employment, healthy lifestyles, healthcare access, early childhood education and other categories. Applications may be submitted at any time and are reviewed six times per year. Additional information can be found on the Web site.
