Description

Headquartered in San Francisco the Health System delivers care to diverse communities in 21 states. It’s the fifth largest health system in the nation and the largest health provider in California. The Health System’s community grants support organizations that work to improve access to jobs, housing, food, education, and health care for people in low-income and minority communities. Funds are to be used to provide services to underserved populations (economically poor, women and children, mentally or physically disabled, or other disenfranchised populations). Programs or projects must be a response to the health priorities identified in the local Dignity Health hospital’s community benefit implementation plan. The funded projects must comprise three or more partnering organizations and should be linked to the Dignity Health hospital in the community. Instructions on how to apply for funding can be found on the Web site as well as additional information on Dignity Health and the locations it serves.
