Headquartered in San Francisco the Health System delivers care to diverse communities in 21 states. Itís the fifth largest health system in the nation and the largest health provider in California. The Health Systemís community grants support organizations that work to improve access to jobs, housing, food, education, and health care for people in low-income and minority communities. Funds are to be used to provide services to underserved populations (economically poor, women and children, mentally or physically disabled, or other disenfranchised populations). Programs or projects must be a response to the health priorities identified in the local Dignity Health hospitalís community benefit implementation plan. The funded projects must comprise three or more partnering organizations and should be linked to the Dignity Health hospital in the community.
