Description

The Foundation’s Sojourns Program supports palliative care initiatives in Idaho, Oregon, Washington, and Utah. The Foundation is committed to transforming health care and improving end-of-life care in collaborative and innovative ways. The Program helps to ensure that palliative care is available to all patients with advanced illness. The program seeks to improve patients’ and family members’ satisfaction with overall care; integrate palliative care with local health care systems through partnership between hospitals and their communities; reduce patients’ pain and suffering; and promote sustainable business models for hospital-based palliative care. The Foundation awards two types of grants: Implementation and Innovation. Implementation grants of $125,000 to $250,000 are awarded to nonprofit and public hospitals, hospices, and other healthcare organizations that either do not offer a formal palliative care program or have not had an active palliative care program in the past two years. Innovation grants of up to $250,000 are awarded to nonprofit and public hospitals or other healthcare organizations that provide palliative care services and seek to close gaps in local health care systems via partnerships with other organizations in their communities. Proposals are accepted on a rolling, year-round basis. Additional information can be found on the Web site.
