DescriptionThe Foundation is accepting letters of intent for funding for home- and community-based healthcare and health services that address the needs of the medically underserved in the metropolitan Chicago region, specifically Cook, Lake, McHenry, DuPage, Kane, and Will counties with an emphasis on Chicago. The Foundation considers requests for program, operating, and capital grants to support home health care services, community and school-based services, primary care and chronic disease management, and health promotion. In deference to the Foundation’s roots as the Visiting Nurse Association of Chicago, priority will be given to programs in which nurses provide care. All letters of intent and invited full proposals should be submitted through the online grants management system. Additional information can be found on the Web site.
