DescriptionFounded by the chief executives of the Great Atlantic and Pacific Tea Company, the Foundation’s goal is to increase the nation’s capacity to provide effective and affordable care to its rapidly increasing older population. The Foundation is a committed champion of health care, training, research, and service system innovations that will ensure the well-being and vitality of older adults. Grants are made in two priority areas: academic geriatrics and training, and integrating and improving services. Under the academic geriatrics and training category, the Foundation supports efforts at selected academic medical centers and other health settings to strengthen the geriatric training of America’s physicians, nurses, and social workers. Under the integrating and improving services category, the Foundation supports a limited number of sustainable efforts to improve and integrate the system of services needed by older adults and their effectiveness. Letters of inquiry required; proposals are by invitation only. Funding emphasis is on nationally replicable models. Additional information can be found on the Web site.
