DescriptionThe Visiting Nurse Association Foundation supports home- and community-based health care and health services for the medically underserved in Cook, Lake, McHenry, DuPage, Kane, and Will counties in Illinois, with a focus on Chicago. The Foundation awards capital, program, and operating grants to home health care services; community and school-based services; primary care and chronic disease management; and health promotions. Priority is given to programs where nurses provide care. Letters of inquiry are requested. The deadline is for letters of intent. Faxed or e-mailed letters of intent and proposals will not be accepted. Additional information and application instructions are available on the Web site.
